Workaround for missing “health heart”.
One item in the game that is not available in the level editor is the health heart, which can be found near the top of the mountain at Bomb-Omb battlefield and at many places in Bowser's levels.
But a close workaround is using an invincibility power-up star (that also recovers health and oxygen) whose expiry is set to the fraction of a second.
Then it acts in a very similar way.
I can remember seeing this trick in a Nathaniel Bandy SM63 custom level reaction video.
